Online Referral Form
We value your referrals and to help make the process as easy as possible, we have provided an online referral form. Digital images and photos can be either attached and sent electronically, or mailed to our office directly.
If your patients would benefit from three-dimensional imaging to supplement their treatment plan, the Three Rivers Oral and Facial Institute (TROFI) located within our facility can provide full-cranial 3D CT scans. Utilizing the latest in I-CAT technology, a low radiation cone-bean will capture images which will be sent to you on a CD, equipped with all necessary visualization software. You can specify the type of scan on the referral form which can be sent to us in advance, or with your patient to their appointment.
